To ask the Minister of Health to detail (i) any process by which the Framework for Learning and Improvement from Patient Safety Incidents was developed; (ii) the dates of the first six meetings of the Steering Group overseeing the development of the Framework; (iii) any papers tabled at each of these first six meetings; (iv) the date when service users were advised that they were being invited to join as a member of the Steering Group; (v) the first date when a service user attended a meeting of the Steering Group; and (vi) the first date when all three of the service users selected by his Department attended a meeting of the Steering Group.
