To ask the Minister of Health whether cases involving delayed diagnosis of serious congenital conditions are routinely reviewed to identify missed opportunities across maternity, primary care and paediatric services, including how any learning from such cases is communicated to GPs, Health and Social Care Trusts and relevant clinical staff.
