To ask the Minister of Health to detail (i) the number of (a) confirmed; and (b) suspected data breaches recorded within Digital Health and Care, in each of the last five years; and (ii) any safeguards, monitoring arrangements and access controls that are in place to prevent unauthorised access to patient records by health and social care staff.
