I cannot confirm that number because I am told that a bit of duplication was involved in the numbers. I am happy to write to the Member with an update on that. It was a significant number. In fact, one is one too many because Encompass is an incredibly expensive system. It just should not happen. Although the referrals were negative in that the consultant was saying, "I don't need to see your patient", there may have been occasions when the consultant was also saying, "but you may want to consider" some other course of action. Therefore, we should not be, in any way, dismissive of the breakdown in Encompass. The question that I am still asking is this: on top of a system failure with the machinery, was there also human error?
